Jen works to improve the health and well-being of communities across FHC’s catchment through strategic partnerships, integrated care and population health planning. Jen is an advocate for community-based primary health care, in which she has been a leader for more than 15 years using a social determinants of health approach. She is passionate about reducing barriers for those most in need, including refugees and those without status. Jen is LEAN certified, trained in the LEADS Leadership framework and certified in Community Health Leadership by Rotman. Jen holds a master's degree in Public Health from the University of Toronto. Outside of work, Jen keeps busy with her daughter Dylan, enjoys walking her dog and learning to play soccer.
Passionate about leadership and its growth and development within the community healthcare sector, Mireille is also particularly committed to health equity and the social determinants of health. With experience in both the for-profit and not-for-profit healthcare sector, Mireille brings a unique perspective in support of Flemingdon Health Centre’s primary care, chronic disease and mental health and population health and wellness programs. Mireille is trained in the LEADS leadership framework and also obtained her Community Health Leadership Certificate from the University of Toronto - Rotman School of Management. Prior to pursuing professional leadership courses, Mireille attended Toronto Metropolitan University where she completed her Certificate in Project Management. She also holds a Bachelor in Applied Science in Kinesiology from Simon Fraser University. When Mireille is not working, she is busy with her 3 soccer loving kids, and her fishing obsessed partner. She also loves travelling and spending time outdoors.
With a solid foundation in journalism, administration, and leadership, Laura blends her diverse skills and experience with a fervent passion for health equity and the social determinants of health. She emphasizes client-centered processes, leveraging data to drive meaningful change. Boasting over 15 years in public policy, community health, and mental health, Laura remains steadfast in her commitment to supporting teams in implementing transformative quality improvement processes. Her goal is to effectively address health disparities and catalyze substantial, positive transformations within communities.
Laura also co-leads the Latin-American Covid Task Force, diligently focusing on reducing health disparities among individuals and families in the GTA.
Outside of her professional endeavors, Laura is a devoted mother and a passionate soccer fan.
Catherine is a family physician and Medical Director of Health Access Thorncliffe Park (HATP), as well as an Assistant Professor in the Department of Family and Community Medicine at the University of Toronto. She was an emergency physician for more than 10 years and is now a passionate advocate for her family practice patients. She received the 2019 Ontario College of Family Physicians Award of Excellence for her leadership in supporting vulnerable populations. In her role as Medical Director (physician engagement and health systems design) at HATP, Catherine has been establishing strong collaborative relationships between the health, social and educational neighbourhood agencies in order to implement inter-sectoral and seamless delivery of care to patients. She is also Chair of the Board of Directors for the East Toronto Family Practice Network (EasT-FPN), a network of family physicians with a mission to create equitable access to inter-professional care for all family practices.
Neil Stephens has over 14 years of experience at Flemingdon Health Centre (FHC), where he has centred his work around the collective impact and community engagement frameworks. He has had the privilege of serving in developing the South Asian Diabetes Prevention Program (SADPP), which created a tailored population-specific diabetes screening tool. Neil then managed the Population Health and Wellness department at FHC, working alongside teams that designed and implemented transformative health initiatives across FHC's catchment, supporting planning tables and networks.
More recently, Neil supported the startup of FHC’s Community Health Ambassador Program, working alongside 35 ambassadors and Ontario Health Teams (ETHP and NYTHP) to expand access to COVID-19 vaccinations and deliver wraparound population health and primary care support to equity-deserving communities.
Now, serving as Senior Manager of Systems Transformation, Neil is co-designing an integrated chronic disease prevention and management hub in a matrix role with NYTHP. He works closely with partners and community stakeholders to ensure barrier-free access to care for those most in need. Neil can be found in the kitchen concocting new recipes and experimenting with hot sauces when not at work.
Maria is skilled in leading and supporting her diverse, multi-disciplinary teams, fostering an environment where daily interactions become opportunities for learning, collaboration, and problem-solving. Her approach ensures the delivery of exceptional services to equity-deserving clients and community members. Known for her organizational skills, reliability, and effective leadership, Maria is deeply committed to motivating her team to achieve impactful service delivery and promote efficient inter-professional collaboration for underserved communities. A passionate advocate for self-care, Maria enjoys nature walks with her family, finding balance and inspiration in the outdoors.
Licien is a dedicated advocate for marginalized communities, with a deep commitment to breaking down oppressive systems and barriers. She has spent over 14 years working at FHC, where she has contributed to the organization’s mission in various roles within the finance team. As the current Finance Manager, Licien Licien plays a critical role in maintaining the organization’s financial health while ensuring that its financial practices align with and support FHC's mission to create meaningful, lasting change for underserved populations.
Mussarat Ejaz is a seasoned health professional with over two decades of experience in clinical practice and education, dedicated to enhancing community well-being and tackling health inequities. Currently, she serves as Manager of Population Health and Wellness at Flemingdon Health Centre, where she leads a team of Community Health Workers and Ambassadors focused on delivering health promotion programs to diverse, equity-deserving communities in Toronto. Her passion lies in chronic disease prevention, health equity, and utilizing social prescribing to address population health issues. Mussarat has a proven ability to build strong partnerships across the health sector and local organizations. An international medical graduate from Pakistan, Mussarat holds a Fellowship in General Surgery from the Royal College of Surgeons in Ireland (FRCSI), further underscoring her extensive expertise and commitment to health improvement.
Lindsey is an experienced healthcare manager with a Master of Health Administration (MHA), specializing in integrated care and quality improvement initiatives. With a proven track record of leading interdisciplinary teams and driving positive change in healthcare service delivery, Lindsey is dedicated to enhancing care coordination and improving outcomes in complex healthcare environments.
Claudia leads the Human Resources function at FHC, which spans labour relations, recruitment, training & development, organizational culture and employee engagement, employee safety & wellbeing, talent empowerment, succession planning, and organizational development.
Samyuktha is the passionate manager of Flemingdon Health Centre's Primary Health Care Team. With a BScN from Toronto Metropolitan
University and a commitment to a client-centered and social justice approach to all her work, Samyuktha has stepped into leadership and
managerial roles with years of frontline nursing experience informing her approach to community health and wellbeing. With a clear vision of how every individual can contribute to a thriving community, she is proud to uplift a diverse and talented Primary Care Team.
Elissa has an extensive background in the non-profit sector where she has contributed and developed in staff and volunteer roles ranging from community engagement to project management. As a life long learner, she studied Leadership Development, Non-profit and HR Management and holds a PMP certification. She is passionate about purpose driven work grounded in social justice.
Stella has 13 years of experience at Flemingdon Health Centre in various roles and capacities. As a Clinical Program Coordinator, she is the responsible in managing the medical secretaries across both the Flemingdon and Fairview sites. Liasing with all Program Teams in the Primary Care, Population Health and Wellness, Chronic Disease and Mental Health. Had over 14 years of managerial experience in sales, special projects, administration and operations before coming to Canada. Has a degree on Bachelor of Science in Commerce, Business Administration
Ambreen is a dedicated clinical program coordinator with a Master's in Business Administration, focused on enhancing clinet care.
With a passion for improving patient experiences, she strives to implement effective programs and foster strong relationships. In her free time, she enjoys reading and gardening.
Alisha holds a Bachelor's degree in Psychology and Post-Graduate in HR Administration. She brings a blend of people-focused leadership and operational efficiency to facilities management. She is passionate about creating efficient, safe, and well-maintained environments that support FHC's organizational success.
With over 10 years of practical experience as a physician, Sonia brings valuable insights as an immigrant mother of two children, healthcare leader, and community advocate. She is deeply committed to enhancing healthcare experience for communities, driven by a belief in equal opportunities for all. Her recent Master’s in Health Administration in Community Care from Toronto Metropolitan University led her to Flemingdon Health Centre, where she passionately contributes to uplift and develop the community.
She draws her energy from singing, cooking and outdoor activities with her family.
